Essential Functions

  • Advocate for assigned individuals, attend all meetings, and maintain relationships.
  • Implement and follow all Hope Family Partners policies.
  • Develop service plans with the treatment team.
  • Maintain client records.
  • Provide in-program support Mon–Fri and be available after hours/on-call.
  • Supervise and schedule staff.
  • Give feedback and conduct staff evaluations.

Responsibilities

  • Assist with Individual Program Plans (IPP).
  • Maintain accurate records and reports.
  • Communicate with managers, clients, and legal guardians.
  • Discipline staff as needed.
  • Audit documentation, mileage, and more.
  • Train staff on clients and activities.
  • Provide direct support as needed.
  • Coordinate appointments.
  • Oversee MARS, Menus, Schedules, and more.
  • Minimum 5 days of program support weekly.
  • Other duties as assigned.

Physical Requirements

  • Stand or sit for long periods.
  • Stretch, stoop, lift (up to 50 lbs).
  • Shovel and clear pathways.

Qualifications

  • High School Diploma or GED.
  • 1–2 years of experience supporting individuals with disabilities.
  • 1–2 years of management or training experience.
  • Valid Maine Driver’s License, registration, and insurance.
  • Certifications: DSP/CDS, CRMA, FA/CPR, Behavior Management.
  • Vaccination records: Hepatitis B, Varicella, MMR.
  • Work permit if required.

Job Summary

The Shift Leader is responsible for the overall wellbeing of the program by cultivating an environment that fosters independence within a safe and clean environment while encouraging the enhancement of positive relationships with individuals with Intellectual Disabilities or Autism Spectrum Disorder supported by Hope Family Partners.

Shift Responsibilities

  • Ensure DSP job tasks are completed:
    • ISP Data
    • GER if applicable
    • T-Log
    • Medication administration
    • Punching In and Punching Out
  • Ensure every individual is supported according to their individual support plan
  • Report to the management team

Primary Responsibilities

  • Manage and support direct care staff to allow them to perform their roles safely and effectively
  • Support your Residential Coordinator and Home Manager to run your workplace according to HCBS standards
  • Ensure the home is safe and clean
  • Ensure medications are refilled and orders are up to date
  • Ensure all positive support plans and client documentation is completed by the end of all staff shifts
  • Ensure all incident reports/GERS are completed
  • Be able to speak and write clearly in English
  • Ensure monthly reports are completed by the proper due date
  • Report all issues to your supervisor by text and/or email

Physical Requirements

  • Ability to do standing/sedentary tasks for long periods of time
  • Ability to perform range of motion tasks including stretching, stooping and lifting
  • Ability to lift 50 pounds
  • Able to shovel and clear pathways

Qualifications

  • High School Diploma or GED
  • Experience supporting people with disabilities, in good standing, at Hope Family Partners for 1 year
  • Valid Maine Driver License
  • Valid CRMA, CDS/DSP, First Aid/CPR and Safety Management
  • Work permit if required

Essential Functions

  • Advocate for assigned individuals, i.e., attend all meetings, and maintain relationships within the home and community.
  • Attending to individuals' personal care needs (may include lifting and assisting individuals in and out of wheelchairs and maintaining good hygiene).
  • Transporting individuals, as needed, using company or personal vehicles to community outings.
  • Transferring from one assignment to another as requested by your supervisor.
  • Cooking and cleaning.
  • Passing Medications as prescribed by a physician or NP.

Responsibilities

  • Assist in developing, implementing, and monitoring Individual Program Plans (IPP).
  • Maintain accurate records including daily logs, individual files, reportable events, and incident reports.
  • Maintain open communication with team members regarding goals and progress of program participants.
  • Follow all schedules as set by the participants and HFP Support Team.
  • Ensure any facility where individuals use house and yard is kept clean and safe.
  • Maintain professional boundaries with clients and clients' families.
  • Clear all emergency pathways for a clear exit in case of emergency.
  • Support clients in their preference in religious services and customs.
  • Meal prep and cooking (use gloves if needed for religious reasons).
  • Transportation of clients.
  • Supporting clients in money management.
  • Employment supports in transporting clients.
